I think @Leon means "have logged 600 issues in the tracker" - experience shows at least 10% will be dupes, and 10% will be "Behaviour as expected"... a chunk will be "the requirements are wrong" some "the test case is wrong" leaving the balance as "proper bugs"

Welcome to SQA... I'm not convinced by your suggestion that SCA should be under QA - IMHO, SCA should be done by the developers, with the objective of minimising the number of violations BEFORE RELEASE rather than as a second iteration.

Terminology is, as you say, key. I'd use Accepted for this status. To me Open is the antonym of Closed rather than a specific stage. But I agree with the extra step. In Progress means, er, in progress

It very much depends how the project looks at it. If done in the right order, it can be VERY beneficial and cost effective. If looked upon as a box ticking exercise, to be left until later, it is a complete waste of time. Sadly too many companies think "box ticking"
