comment How to create a 'quality-centric culture' within a team/organization?
Sorry for the late answer. Well, for the developers Quality is not really a serious topic. Some are at least making Unit tests, but when it comes to collaborating with testers ... or removing technical debt, refactoring etc. no one really sees the point.

comment Is it common to track defects found during code reviews?
Thanks for the answer! Why do you try to avoid commenting pure technical debt?

Thank you for your answer! What about Design Defects? Lets say one developer built in some code smells; so there is no functional defect, but it causes technical debt, as ,for example, this part of the system is harder to extend.

comment How to finish the testing in the same iteration/sprint?
Thanks for the answer. Developers are writing automated tests (Unit + Acceptance) already and those are used for regression. In my opinion, there should still be a tester for manual tests ("Big Picture" Test Cases,Exploratory Testing). How do you handle manual testing?
